How to Apply to CARES!

Contact your desired Community Based Organization Core Lead.

Next, the core lead will connect you with a community-based organization outreach staff member who will conduct the CARES pre-screening (are you eligible for CARES?), surveying, and pre-enrollment (what do you need when you show up to a enrollment appointment?).

Once the pre-enrollment and survey is completed, ideally, you will be referred to the health center (Depending on site availability).

A enrollment specialist from a Community Health Center will call you to schedule enrollment appointment.
CARES Enrollment

Potential patients go to their enrollment appointment (In-person or over the phone) to the Health Center where they want to receive care and bring:

Proof of Income (Paystub/Taxes)​

If they are paid in cash, they will sign a self-attestation form.​

Proof of Identity (ex: Matricula, CA ID, Passport)

Proof of Residency (Utility Bill)

Appointments take 10-20 minutes​

Submitting Application ​

​CARES application are emailed to Contra Costa Health Plan by enrollment counselor for final review.​

Application that are submitted BY the 23rd of the month, can expect CARES coverage to begin the 1st of the month (if approved).

Application submitted AFTER the 23rd of the month, can expect coverage to begin the 1st of the following month.

Welcome Letter and CARES Card

If approved by Contra Costa Health Plan CARES participants will receive a Welcome Letter and CARES Card from Contra Costa Health Plan. (May take up to 30 days)

Enrolled and Approved

CARES participants will have coverage for 12 months.​

The CARES participant will no longer be eligible for CARES if they move out the county or are eligible for other health insurance (restricted Medi-Cal does not count).
